Position Overview:
XCEL Home Health is seeking Licensed Practical Nurse (LPN) for an exciting Travel or Local job in the locations identified below. The Licensed Practical Nurse (LPN) provides nursing care and support to clients under the supervision of a Registered Nurse (RN) and in accordance with Agency policies, State, and Federal regulations. The LPN assists in implementing the client’s Care Plan, provides direct care, monitors client conditions, and reports changes to the supervising RN and/or physician.

Location(s): BURTONSVILLE / SPENCERVILLE / POTOMAC, MD
Reporting Relationship:
- Reports to the Registered Nurse and Agency Manager/Administrator.
Qualifications/Prerequisites
- Graduate of an accredited school of practical nursing.
- Current LPN license issued by the State Board of Nursing.
- Minimum of one year of nursing experience, preferably in home care or long-term care.
- Maintains good physical and mental health with pre-employment health clearance.
- Must pass background check and drug screen.
- Licensed driver with a clean driving record and insured vehicle (if required for client visits).
Responsibilities/Activities
- Provide direct client care in accordance with physician orders and Care Plan.
- Monitor and record vital signs, symptoms, and client conditions.
- Administer medications and treatments as delegated by RN or physician.
- Report any changes in client condition to supervising RN.
- Assist clients with activities of daily living (ADLs) as needed.
- Support health teaching for clients and families.
- Maintain accurate and timely clinical documentation.
- Participate in in-service training and staff meetings.
- Comply with infection control, HIPAA, and agency policies.
